To obtain an academic transcript of your years with the Napa Valley Unified School District you need to send:

1. The name under which you attended school;
2. The name of the school you last attended at NVUSD;
3. Your birth date;
4. The year you graduated or left school;
5. A day time phone number;
6. An address where you want the transcript sent;
7. Your signature on the request;
8. A check or money order for $2.00 per transcript payable to NVUSD.

Mail above information and payment to:

   Napa Valley Unified School District
   Instructional Services
   2425 Jefferson Street
   Napa CA 94558
